But what does the ISO 9000:2015 standard say about what a nonconformity is? A nonconformity is simply the non-compliance with a requirement that the quality management system of a given organization has previously determined. This can include product defects, process errors, non-compliance with quality standards, complaints from dissatisfied customers, among others.

Each nonconformity should be recorded and properly documented where it has occurred. Non-conformities can be divided according to their origin. Following this premise, they can have an external or internal origin. On the other hand, when the origin is internal, these nonconformities are associated with non-compliance with the organization’s own procedures or requirements.

Non-conformities can also generally be of two types depending on their severity: a minor non-conformity (Minor NC) or a major non-conformity (Major NC).

On the one hand, Minor NCs do not have a significant impact on the organization’s quality management system.

Major NCs, however, are related to problems of considerable significance that affect the quality, safety or efficiency of a product, service or process as a whole. These nonconformities are usually related to critical issues, such as safety failures, non-compliance with key specifications, defects in the final product, process control problems or systemic problems that jeopardize the integrity of the quality management system.

Major NCs can also derive from a Minor NC that has not been resolved within a preset time frame or from several Minor NCs that are related to the same process.

    When the presence of a minor nonconformity (Minor NC) is determined, a certain allowance is allowed that is properly recorded and documented. The main purpose of recording minor nonconformities (Minor NC) is to keep a record and maintain a history of the problems that have been identified, which can be useful for monitoring and continual improvement of the quality management system.

    On the other hand, when a nonconformity is classified as a major nonconformity (Major NC), it is essential to perform a detailed assessment of the severity and impact of the nonconformity. In this way, the magnitude of the problem is understood and serious consequences, such as loss of customers, non-compliance with regulations, damage to the company’s reputation or even health and safety risks, are to be avoided. This will prioritize the corrections and corrective actions to be taken to effectively address the nonconformance and avoid similar problems in the future.

    Corrections are a company’s first response to a nonconformity. These corrections can occur before, during or after the nonconformity has been detected. Their primary objective is to address the immediate problem and eliminate the nonconformance itself. They do not require a thorough review or detailed root cause analysis. Corrections may be temporary measures or quick fixes to resolve the problem immediately and prevent it from escalating. Not all nonconformities will require corrections, as some may be minor or not have a significant impact on the quality of the product or service.

    In contrast, corrective actions represent a planned, long-term response to a nonconformity. These actions are carried out after a detailed review and analysis of the nonconformity to identify its root cause. The main objective of corrective actions is not only to eliminate the nonconformity itself, but also to address and eliminate the underlying cause. This means that measures are taken to prevent the nonconformity from recurring in the future. Corrective actions have a preventive focus and are designed to improve processes and systems, which contributes to continuous quality improvement.

    In order to detect and analyze the root cause of a nonconformity, there are several methodologies that are very useful.

    One of these methodologies is the Ishikawa diagram, also known as the fishbone diagram or cause-effect diagram. This graphic tool allows to identify and visualize all possible causes that could have contributed to the nonconformity, helping to understand the problem in a more complete way.

    Another useful methodology is the 5 Why’s technique. In this analysis, a series of “Why?” questions are asked iteratively to delve deeper into the underlying causes of nonconformance. This technique promotes a deeper and more systematic approach to uncovering the reasons behind the problem.

    8D Analysis is a structured, eight-step process for addressing and resolving quality problems. This method includes forming a team, describing the problem, implementing corrections and corrective actions, and verifying their effectiveness.
