What is FMEA?

FMEA or Failure Mode and Effects Analysis is a methodical and collaborative approach to identify risks before they materialize, convert them into prioritized actions and review them on an ongoing basis.

It is a document that is used and maintained throughout the product/process life cycle. It is recommended to start from base FMEAs (previous knowledge) and specialize them by process families to accelerate deployments and homogenize criteria.

Types of FMEA

FMEA Design

It is applied from the product definition phase to prevent failures before manufacturing. It analyzes functions, requirements, tolerances, materials and conditions of use to identify possible failure modes and their effects, prioritizing them according to their severity, frequency and detectability.

FMEA Process

Prevents failures in manufacturing and assembly. Reviews the flow of operations, machine parameters, tooling, working methods, human-machine interaction and means of control. Defines prevention and detection measures (error proofing devices, measurement limits and frequencies) and reaction to deviations.

FMEA System

Provides a global view for complex products or installations. Evaluates interfaces between subsystems (mechanical, electronic, software), supplier dependencies, logistics and functional safety. Considers fault propagation, redundancies and degraded modes to ensure reliability of the complete system.

What does FMEA consist of?

It is composed of 7 steps that replace the NPR/RPN with the Priority for Action (PA) to decide what to improve first, and requires senior management acceptance of residual risk and mitigation measures.

  3. Functional analysis.
  4. Failure analysis.
  5. Risk analysis.

    Why apply the FMEA?

    Applying FMEA reduces the Cost of Quality, shifts effort to prevention, reduces rework/scrap and avoids external failures with reputational or legal impact. In management terms, it converts risk into measurable operational decisions.

    How do I apply an FMEA?

    1. Planificación y preparación

    Define the Intent, Time (when and with what milestones), Multidisciplinary Team, Tasks and Digital Tools to be used. Document the scope and master data.

    Break down process → steps/stations → elements (machine, method, material, material, labor, measurement, environment) to order the analysis.

    3. Análisis funcional

    It declares functions and requirements at each level, the non-fulfillment of the step function defines the failure mode and that of the process its effect.

    Relate Effect-Mode-Cause (what the customer perceives, how it manifests itself, why it happens) and rely on historical plant/supplier data.

    Assesses Severity, Occurrence and Detection based on current prevention and detection controls; prioritizes with PA (High/Medium/Low) what to do first.

    6. Optimización

    Define measurable actions with responsible party and date; close the cycle by evaluating effectiveness and updating the record.

    7. Comunicación

    Share results, retain know-how and link changes with Control Plan, instructions and training.

    Importance of FMEA today

    In an environment where suppliers, references and deadlines change almost weekly, waiting until the end to see what happened is more costly. The FMEA brings the conversation forward to the right time, before manufacturing. It helps design, process, purchasing and plant teams share a common risk picture, agree on what is critical and define how to prevent rather than just correct.

    FMEA is not paperwork, it is a way of working. When connected to the operation through Control Plan, guidelines, audits, SPC and management of nonconformances and corrective actions, decisions become faster and less debatable because there is data, accountabilities and dates. Start simple with product families or key operations, keep it alive every time something changes and allow the learnings to accumulate.
